The Young at Heart Senior Center & Community Center offers programs and services for residents of all ages; child daycare, education outreach, senior meals, in-home health services, and public-use space for meetings and events.
Monday until 8:00 pm, Monday night dinner starts at 6 pm
- Young at Heart Senior Center Caregiver Program
- The Caregiver program is aimed to assist at-home caregivers 18 and older that are caring for a qualified receiver 60 yrs or older or a person 18 and older taking care of a person with Alzheimer’s disease or related Dementia, to aid the caregiver in keeping their care-receiver in the home.
- The Caregiver program provides light homemaking, respite services, and personal care, along with assistance, information, and education on caregiving and Alzheimer’s Disease/Dementia.
- Our respite services are designed to assist the caregiver with that much-needed break; it allows the primary caregiver to hit the “reset” button which helps them continue to provide care to loved ones.
- Caregiver Support: At this time we do not have a support group. Monthly information is sent out to caregiver clients on related conditions/issues.
- Senior Companion Program: Make a difference in the lives of home-bound seniors. If you are age 55+, you can earn money while enjoying the companionship of home-bound seniors. Senior Companions are needed in Rock Springs and Green River.
- Early Learning Center
- Strong partnerships with parents, use of emergent curriculum, indoor and outdoor gross motor space, developmentally appropriate learning, an “open door ” policy for parents, and the utilization of technology are key elements to the Early Learning Center which provides a fun, safe, and nurturing learning environment. In addition, the ELC provides a nutritious breakfast, lunch, and afternoon snack daily. These meals are prepared by the kitchen staff of Young at Heart Senior Center.
- Home Delivered Meals Program by Young at Heart Senior Center

The senior center Home Delivered Meals Program serves Sweetwater County individuals who are unable to prepare meals for themselves and are primarily home-bound. Meals are delivered daily, Monday through Friday, from 11:30 a.m. to 1 p.m. Frozen dinners are available (upon request) for nights and weekends. The quality and taste of food is well known. These folks really know how to cook!
If you or someone you know requires these services, give us a call. We would be glad to help.
Volunteers are always welcome for the delivery of meals. This is a rewarding and fun way to spend a couple of hours a week.
- A Special Diets Program is prepared at the hospital for those with special dietary needs, as prescribed by your doctor. Be sure to remind us about this.

The goal of the YAH Home Health Care Agency is to help patients recuperate at home and to remain independent at home for as long as possible. As insurance companies strive to reduce costs, standards have been set for how long a person may stay at the hospital for certain procedures or illnesses. Unfortunately, not everyone recovers at the same speed.
Rather than have people go home from a hospital to try to care for themselves or to have to go into a rehabilitation facility, home health care can provide skilled nursing services along with personal care from certified nursing aides, physical therapy, and other services to the person in his/her own home. Medicare, Medicaid, and private insurance will often cover the complete cost of home care.
Numerous studies have shown that people who are ill, have suffered an injury or who have had recent surgery, recover more quickly in their home environment. The cost for nursing home care can be prohibitive for someone who just needs assistance with bathing, dressing, or light meal preparation. Sometimes simple strengthening exercises are all that is needed to help someone regain their independence.
We will provide care to adults 18 years and older. A doctor’s order is required for services. Our office can assist you in obtaining an order.

In-Home Services provides homemaking support to adults age 18-59 who are disabled and to those age 60 and above who are unable to perform normal daily tasks independently due to multiple physical and/or environmental conditions.

Eligibility consideration included but is not limited to, physical, emotional or cognitive functions, and possible abuse or neglect.

In-Home Services is a cost-sharing program funded jointly by the State of Wyoming, Sweetwater County, the United Way of Southwest Wyoming, Community Services Block Grant, and patient resources.

Fees are determined on a sliding fee scale according to patient income.
