Wyoming’s Care Management Entity serves youth with complex behavioral health conditions.

Through our collaboration with the Wyoming Department of Health, Division of Healthcare Financing (Medicaid), we coordinate care and service delivery through the High Fidelity Wraparound model to support the success of children, youth, and their families in their homes, schools, and the community.  Wraparound is a team process for families that helps youth with behavioral health conditions stay in their communities, in school, and out of trouble.  It is focused on the family’s goals. It helps the family put together a plan they create that gets all of the other entities in a family’s life working on the same plan, instead of contradictory plans.  Wraparound also helps families martial their natural supports- like relatives and friends, to work together with them to support the youth. We train, certify, coach, mentor, and monitor the wraparound providers and their work, which is free to the provider.  Magellan also authorizes payment of providers for wraparound services.
