Home Life Care is a home and community-based waiver program providing exemplary care to adults with developmental disabilities. We serve adults with disabilities through the Wyoming Medicaid DD (developmental disabilities) Waiver.
We offer Residential Habilitation and Day Habilitation services. Our primary emphasis is that ALL participants are treated with dignity and respect. We support participants’ choices and assist in helping them meet their goals. We create an environment that complies with the requirements of each participant’s Individual Plan of Care.
- Residential Habilitation: We provide a home environment and staff to assist participants with home-life skills. We ensure a safe and clean environment with all the support necessary for daily life activities in the home and community. We provide transportation for participants to attend all appointments and activities in the community in which they have an interest. Each of our staff is trained in CPR, First Aid, and Medication Assistance. In essence, we ensure that all aspects of a participant’s care is completed thoroughly with a total focus on their needs and choices.
- Day Habilitation: In our Day-Hab program we offer a wide variety of activities at our center that are specifically designed to assist in skill building. Manual dexterity, following rules, and problem-solving activities are provided. We also ensure that participants are able to attend community functions if they so desire. We have activities including going to the movies, dining out, and bowling to name a few. Community outings are available as well. Our Day-Hab is available from 9:00 AM – 4:00 PM Monday through Friday.
